Narrative and Financial Reporting
During
the LSTA Grant period, the grantee is required to submit LSTA
reports. The LSTA reports are made up of one or all of the following:
Narrative Activity Worksheets, Financial Reports, Final Narrative
Reports, and supporting documents for the project such as surveys, printed
materials, articles, photos and/or other products that are created.
Your grant contract will indicate which reports are required, and how
often they must be submitted.
The grid below displays the typical report type and schedule required for the three LSTA Grant Types.
Local
Library and Statewide Library Service Grantees must submit Activity
Worksheets, a Final Narrative Report, and Financial Reports.
Fiscal Agent Grantees are only required to submit Financial Reports.

Narrative Reports: Activity Worksheets
Activity Worksheets are narrative reports. These are required for Local and Statewide Library Service grants.
You only need to submit the type(s) of Activity Worksheets that apply to your project’s activities.
Instruction Worksheet (PDF) | Involves an interaction for knowledge or skill transfer. |
Content Worksheet (PDF) | Involves the acquisition, development, or transfer of information. |
Planning and Evaluation Worksheet (PDF) | Involves design, development, or assessment of operations, services, or resources. |
Procurement Worksheet (PDF) | Involves
purchasing facilities, equipment/supplies, hardware/software, or
other materials (not content) that support general library
infrastructure. |
Due dates are listed in Appendix C of your contract.
Submit all reports to
ra-LSTA@pa.gov. In your email subject, list:
- [YOUR LIBRARY NAME] LSTA Activity Report, Grant # [NUMBER FROM CONTRACT].
Narrative Activity Worksheets report on how your project was implemented. Each Activity Worksheet summarizes:
- What activities have been completed, including data;
- What you have accomplished;
- What problems you have encountered; and
- Project outcomes.
Required Surveys
Some grant-funded activities (Instruction, Content, or Planning and Evaluation) may require you to conduct surveys. When required, surveys may be conducted electronically or via paper.
Submit your aggregated survey results to ra-LSTA@pa.gov. In your email subject, list:
- [YOUR LIBRARY NAME, SURVEY NAME] Grant # [NUMBER FROM CONTRACT]
Final Narrative Reports
A Final Narrative Report is required of all LSTA projects.
Due dates are listed in Appendix C of your contract.
Submit Final Narrative Reports to
ra-LSTA@pa.gov. In your email subject, list:
- [YOUR LIBRARY NAME] LSTA Narrative Report, Grant # [NUMBER FROM CONTRACT].
The report provides:
- A project abstract;
- Project accomplishments, data, and outcomes;
- A list of participating organizations; and
- Best practices that may have emerged during the project.
The
Final Narrative Report’s information and data are reported to and
reviewed by the Institute for Museum and Library Services (IMLS) in
OCL’s annual State Program Report. Once approved, information about all
LSTA-funded projects is publicly available on the IMLS website.
Financial Reports
Financial Reports are required of all LSTA projects.
Local
Library are required to submit
copies of invoices, purchase orders, etc. to support any charges made
to the grant.
Due dates are listed in Appendix C of your contract.
Submit Final Narrative Reports to
ra-LSTA@pa.gov. In your email subject, list:
- [YOUR LIBRARY NAME] LSTA Financial Report, Grant # [NUMBER FROM CONTRACT].
Each Financial Report lists all actual expenditures to date along with an explanation of charges within each budget category.
It addition, it reports local matching funds spent in support of the
project to date. The local match is the portion of total project costs
contributed in support of the project. These funds may be from local or
private sources, including corporations and foundations.
